Here you submit documents from the medical practice. Make 1 pdf file which you name as follows: surnamename.pdf

Scan your REPORT OF COMPLETED PRACTICES.

recommended procedures to perform/see/assist – 0 – 100% can be completed.
Page 4+5 - attendance 80 hours /must be written the total 80 hours there/.
Signature + stamp of your mentor nurse.
Page 6 - for Czech students ONLY.
If you do your practice abroad DO NOT FORGET TO ADD your "CONSENT of the HOSPITAL ABROAD with the practice" to this pdf file.Before your scan the logbook write your name on the top of these pages - see the example.
Drag the pdf file using Add Submission and SAVE CHANGES.
After checking of the document, you will receive a credit to the SIS.
If there is any problem with your document, you will be notified to correct it and resubmit.
Deadline of your online enrollment is September 14th.
Therefore I highly recommend you to submit your logbook in advance - in case there are any doubts I need to contact the hospital ABROAD!!!!!
