      Submission of Patient Care Medical Practice Documents

      Please upload one PDF file containing all required documents from your medical practice.

      Documents to include

      Report of Completed Practices

      Page 1 – Mandatory procedures

      • Performed procedures: 100% must be completed.
      • Observed/assisted procedures: 100% must be observed or assisted.

      Page 2 – Recommended procedures

      • Recommended procedures to perform, observe, or assist: completion may range from 0–100%.

      Pages 3–4 – Attendance

      • A total of 80 hours must be completed.
      • The total number of hours (80) must be clearly stated.
      • These pages must include the signature and stamp of your mentor nurse + the hospital(or department).

      Students completing their practice abroad

      If you completed your clinical practice abroad, do not forget to include the hospital's official Consent for Clinical Practice in the same PDF file.

      Before scanning

      Please write your full name at the top of all required pages before scanning them (see the example provided).

      Uploading your file

      1. Click Add submission (at the bottom of this page)
      2. Upload your PDF file.
      3. Click Save changes.

      Assessment

      After your documents have been reviewed and approved, you will receive your credit in SIS.

      If there are any problems with your submission, you will be contacted and asked to correct the documents and upload a revised version.

      Deadline

      The deadline for online submission is 15 September, except for students completing their internship in Hradec.

      We strongly recommend submitting your logbook well in advance of the deadline. If there are any questions regarding your documents, we may need to contact your hospital abroad, which can take additional time.
